Upload proof of treatment pricing

Submit an itemized receipt from a cosmetic, dermatology, plastic-surgery, or medspa provider (PDF, JPEG, PNG, or WebP). TxTrust automatically detects and redacts patient identifiers (names, member IDs, SSN, phone, email, addresses) from extracted data. For best results, obscure obvious PHI on the image before upload. Critical PII (SSN, payment card) will reject the receipt for re-upload.

You can upload three document kinds:

Treatment receipt — proof you actually paid (highest value for the verified pricing registry, earns points on publish)
Treatment quote / estimate — what the clinic quoted you (appears on the provider profile as advertised pricing)
Provider price sheet / menu — the clinic's published service list (populates the provider profile's full menu and the advertised-vs-verified accuracy comparison)

The platform auto-detects which kind your upload is — no need to select. Quotes and price sheets skip the confidence-scoring path (they aren't records of a transaction) but still feed the provider profile pages.
